Repair hair transplant

Repair of poorly/wrongly performed hair transplant requires both experience as well as a meticulous approach on part of the doctor.
The hair transplant doctor should have the compassion and the willingness to be able to help the patient restore his confidence in the hair restoration procedure as well as lead his life free from the restrictions that a wrongly performed hair transplant imposes.
At Dr. A's Clinic, we are justifiably proud of the fact that we have managed to successfully repair some of the worst hair transplants performed elsewhere.
There are numerous factors that have to be considered while suggesting a repair strategy for the particular patient.
Each patient has to be considered as an individual case in need of personalized attention.
The mistakes of wrongly performed hair transplant vary greatly. So do the repair strategies.
There are, however, some common issues that can be surmised thus.

Psychological factors :
It is of paramount importance to understand that the person who has gone through a wrongly performed hair restoration procedure feels a sense of emotional betrayal. It takes a lot of courage on his/her part to research and subject himself to a repair procedure.
Previous hair restoration mistakes – whether it is due to ill advised scalp reductions, flap transfers etc.

  • Inappropriate hair line placement (too broad or to low a hair line ).
  • Wrongly directed hair grafts.
  • Hair growing at wrong/unnatural angles.
  • Pitting, cobble stoning, ridging or other scarring in the recipient areas.
  • Unsightly/cosmetically unacceptable scars in donor areas.
    All these points need to be addressed at the start of the repair procedure.
A detailed discussion of what the repair procedure will entail AND its limitations is essential.

(A) Wrongly angled and wrongly directed follicular unit grafts .
Repair strategies may range from...
  • Extraction of the follicular units coupled with reimplanting them in the correct direction (if the number of wrongly angled grafts is less).Camouflaging the grafts with a multitude of properly transplanted grafts.

Problems commonly encountered in such a case is that the wrong angles of the previous hair transplant makes it impossible to ensure a correct or even a compromise angle without a chance of damaging the previous hair grafts.
At Dr. A's Clinic we take special care to minimize such damage while making the new recipient sites.

(B) When plug/ standard grafts (containing 8-20 hair each)
OR
Minigrafts (containing 5-8 hair each) are used in the initial wrongly performed hair transplant, the option available include...

  • Excision of the offending plug grafts (the psychologically traumatized patient may want to just get rid of the evidence of hair transplant).
  • Excision of the plug or minigrafts and dissecting them into individual follicular units that are then retransplanted.
  • Thinning out the plugs. That is, extraction of a fraction of the individual follicular unit from the plug grafts. This, coupled with transplanting additional follicular unit micrografts in the area, leads to a more natural appearance.
  • Camouflaging the plug or mini grafts among ”a sea of ” follicular unit micrografts.

(C) Too broad or too low a hair line.
Too broad or too low a transplanted hair line (including when grafts have been placed in appropriately in the temporal recessions), requires a multiple step approach.
The first step will usually involve excision of the wrong hair grafts placed in the previous hair transplant procedure.
The hair in those grafts are, of course, transplanted in a more appropriate area of the recipient zone after excision.
The excision wound is sutured close, if the graft is too large;
OR
If the graft is a follicular unit , the donor sealing protocol is performed.
The second step will involve construction of a new, correctly placed and designed hairline.
The 2 steps may be performed at a single visit (in some patients) but usually require 2 separate procedures.

(D) Other problems
The problems with a wrongly performed hair transplant are not limited to the OBVIOUS damages it has caused.
The obvious damage includes the grafts growing at wrong angles, directions, densities etc., & cosmetically unacceptable overall appearance of the hair transplants.
The less obvious damage includes the depletion (and in some cases) outright wastage of the precious donor hair follicles.
The donor hair follicles are limited in number. It is hard for the doctor performing a repair procedure to figure out how much donor wastage has occurred during the previous wrongly performed hair transplant.
He can only get a rough idea based on...

  • Too long a donor scar for the number of grafts transplanted.
  • Too thin a density in the recipient area for the number of grafts transplanted.
Wastage or damage to the donor grafts limits the scope of the repair possible.
The repairing doctor has, therefore, to plan an outcome that will be most pleasant and cosmetically acceptable given the reduced number of graft available.
