Factors to Consider

Dr. A's Clinic has successfully repaired some of the worst hair transplants performed elsewhere.  Repair cases are unique and there are numerous factors for the patient to consider. 

Badly performed hair restorations typically exhibit the following characteristics

  • Unnatural growth direction (ex: hair follicles in the crown area pointing straight up instead of a natural circular pattern)
  • Hair plugs
  • Inappropriate hair line placement (ex: too broad or too low)
  • Pitting, cobble stoning, ridging and scarring in recipient areas
  • Unsightly scars in donor areas
  • Unnaturally low density
  • Scalp reductions and flap transfers

In addition, victims of badly performed hair restorations often feel a sense of betrayal.  To go through another hair restoration procedure requires a lot of courage on the patient's part.


Treatment Options
Unnatural Growth Direction

  • Large graft count: Extract incorrectly angled follicular units and re-transplant them in the correct direction
  • Fewer graft count: Camouflage incorrectly angled grafts with a multitude of properly transplanted grafts from donor supply

Problems encountered:
While replacing follicles in a new direction, there is a risk of follicular transection due to angles crossing.  At Dr. A's Clinic, we take special care to minimize such damage while making the new recipient sites.

Hair Plugs

  • Permanent plug removal: Erase all hair transplant evidence
  • Plug dissection: Extract entire plug, dissect into individual follicular units, then re-transplant individual follicular units
  • Plug thinning: Extract individual follicular units from the plug, then re-transplant with additional follicular unit grafts from donor supply
  • Camouflage plugs: Flood areas in-between plugs with additional micro grafts from donor supply

Inappropriate Hair Line Placement
Hairlines too broad or too low require a two-step approach.

  1. Re-transplant incorrectly placed hair grafts to a more appropriate hairline location.  Depending on size of excision, the wound is sutured or allowed to heal on its own.
  2. Fill-in/strengthen new hairline with grafts from donor supply

Donor Supply Wastage
To estimate the number of donor grafts destroyed, we analyze the following variables. 

  • Too long a donor scar for the number of grafts transplanted
  • Too thin a recipient area density for the number of grafts transplanted

Donor supply wastage limits the scope of repair possible.  Therefore, a patient must determine what treatment options are most cosmetically acceptable given the reduced donor supply available.
