Part 1 – HAIR FOLLICLE:

The first term you will come across is hair follicle
A hair follicle is a complete organ in itself composed of different types of tissues.
The hair we see on top of our skin is dead keratin. However, there is a complex multi tissue function below the skin that ensures that the dead keratin is not only extruded but it also comes out in shape of a hair.

The base of the hair root consists of the Dermal Papilla (DP), where cells rapidly multiply. DP is surrounded by 2 hair root sheaths. These sheaths form a tube going from the DP to the surface of the skin opening in form of a pore (infundibulum).
The cells formed during rapid multiplication at DP are pushed up through the sheaths in form of a tight cylinder. These cells are rich in keratin.

By the time these cells reach the skin pore; they are already dead and compressed together due to the pressure exerted by the rapidly multiplying cells below them. These dead cells or keratin are held together by disulfide bonds. These bonds are what ensure the shape and the curl of the hair.

A sebaceous gland is attached to the hair sheaths. The sebaceous gland, secretes sebum. Sebum acts as an oily lubricant that helps the keratin/hair to emerge smoothly from the skin pore without any friction.

The Dermal Papilla is surrounded by a fine network of blood vessels that transport nutrients and oxygen needed by the rapidly multiplying cells.

A tiny muscle (the erector pili muscle) is also attached to each hair follicle. Exposure to cold or fear makes this muscle contract leading to raised hair/goosebumps.

Hair is one of the most metabolically active organs, growing at stupendous pace.
Thus, any drastic changes in nutrition are often seen to effect hair growth. However, this nutrition related hairfall is usually temporary and can be reversed by following a simple regimen of eating a balanced diet.

Word of Advice For The Young Hair Loss Sufferers

Hairloss Basics – People regardless of their age, gender or status can experience hairloss due to several factors. If you are one of those who are experiencing this problem, it is a must that you know the basics of hairloss to get an idea about how to proceed.

Knowing the basics can help you determine the status of your condition, can give an idea when to seek medical attention, can help you decide which hairloss treatment to use.

Anyone losing hair is prone to anxiety.

The first thing they want is to DO something for it. It’s not about whether the treatment is going to help you or not. It’s more about feeling that one is actively doing something for hairloss.

This is the wrong way to proceed.
Do not rush into any treatment.

Whether you are 20, 40 or 60 years of age, the first thing for you to do is to write down the following things.

1. What is your current age?

2. What is the extent of hairloss in male members on both sides of your family? (Does not matter what age they reached that level of hairloss.)

3. What is the current speed of your hairfall? (slow, moderate or rapid)

4. Have you taken some good clear pictures of the extent of your hairloss? Most people do not like taking pictures that show them in an unflattering light. However pictures are important both for your future doctors as well as for you to gauge your speed of hairloss. Take clear pictures , preferably with digital camera. Don’t use flash while taking pictures. Flash distorts the picture and it become difficult to evaluate the real situation.

Take pictures in indirect natural daylight.

Take pictures with the hair combed back to reveal the full extent of your hairloss. Front view and side view must include till atleast the eyebrows. Eyebrows act as a point of reference for the hairline .The top view must include the complete head (both ears included).

Back view and a view of the donor area of the scalp with the hair parted. These pictures are your baseline. Now repeat the same exercise at regular interval. 2 months interval will be fine.

As you have completed taking some pictures, repeat the entire exercise with your hair wet.

5. Have you consulted your family physician or an unbiased physician about the cause of hairloss? Do so. They will be able to rule out any major factor leading to hairfall.

Hairloss need not always be a DHT influenced male pattern hairloss.

6. Does anyone in your family or friend circle suffer from hairloss? If yes, then talk to them about their experience. They have already travelled some of the path you plan to follow.

HAIR FOLLICLE V/S HAIR FOLLICULAR UNIT GRAFT

Hair does not always grow as 1 hair per place. Hair grows in form of groupings called hair follicular units. A follicular unit is a group of hair follicles as they grow in nature. These follicular units consist of 1 to as many as 4 hair per follicular unit.
In majority of instances, especially, in the scalp, hair grows in groups of 2 or 3 hair per place. The picture below is a random example illustrating the concept of follicular units. Notice how even though the hair follicles splay as they go down, on the skin surface they grow close together in form of a unit.

Hair follicle per follicular unit differs in
– different races
– different areas of the body
– different areas of the scalp

However, as a basic thumbrule, for scalp, 1 follicular unit graft is atleast equal to 2 hair on average.

WHEN THERE IS A LARGE AREA OF BALDNESS, IS IT POSSIBLE TO COVER THE ENTIRE BALD AREA?

This depends on the size of the bald area as well as on the donor area.

FEMALE PATTERN HAIRLOSS OR LUDWIG CLASSIFICATION

Hair loss in ladies occurs due to various reasons. Some common reasons include hormonal disturbances, prolonged/severe illness, crash dieting, chemotherapy, auto immune disorders etc. Hair transplants are not recommended if hair loss is due to the above reasons.

However, hair loss due to androgenic alopecia or any injury, burn or surgery scar can be treated using modern, ultra refined follicular unit hair transplants.

In addition, ladies having a broad forehead by birth (congenitally high hairline) can also lower their hairline.

Females usually have a different type of hair loss than men.

Female pattern baldness is different. In most cases, they do not have large areas of stable balding-resistant hair follicles. The sides and the back of their scalp hairs also tends to thin just as the front and top of the head do. Hormone dihydrotestosterone (DHT) affects most of the areas of their scalp.

Women with traction alopecia (hairloss induced by prolonged and excessive stretch on the hair follicles e.g., when using tight rollers or wearing hair in tight braids). These women usually have an isolated area of their hair that is affected. Thus, they can have hair transplant procedures. Some women have cosmetic surgery (brow lift, facelift etc.) and suffer hair loss around the suture sites. In these cases, hair transplant surgery can help. Similarly women who have suffered trauma from accidents or burns can benefit from hair transplant.

For those women who do not have diffuse alopecia, the diagnosis is simplified and much more direct. These women are classified by the Ludwig classification system. There are three categories; Ludwig I, Ludwig II, and Ludwig III.
Those with a Ludwig I pattern are not candidates for any surgical treatment. They are best treated with topical treatments like tricopigmentation/SMP.
Similarly, females approaching post menopausal phase usually suffer from hair thinning in ideal areas and can be benefited with tricopigmentation. Patients with more advanced hair loss, those who are classified as Ludwig II or Ludwig III categories, are possible candidates for hair transplantation. They are acceptable candidates if they have an adequate amount of donor density. The donor area is the hair found in the back of the scalp.

Difference between strip/FUHTand FUSE/Fue

This article compares the techniques used for hair transplant and healing process for each procedure.

1. strip/FUHT

In strip/FUHT, a strip of hair bearing scalp is extracted from the scalp donor area. The strip varies between 1cm to 1.5cm in width. The strip incision is then closed by dissolvable sutures or surgical staples. The hair above the suture covers it. The scar mark will be visible if you buzz cut or shave off your hair (it is normally not visible under a hairgrowth of 1 to 2 cms)

2. FUSE/fue –

FUSE (Follicular Unit Separation Extraction) is a method of extracting individual hair follicles with microsurgical precision but without scalpel and stitches.It is less invasive, without any stitches and there is no visible scar even if the hair is buzz cut.

In FUSE/fue, the follicular units are extracted one by one in a random pattern. The donor sites are left to heal on their own. There is no need of any closure as the donor sites are very small.

The stitchless FUSE/fue technique enables the use of robust body and beard hair as additional donor hair in suitable patients.

Value for Money :-
Some patients prefer to go for the combination of strip/FUHT and FUSE/fue to maximize the number of grafts by keeping their hair restoration procedure more economical. In this, a strip is taken as usually from scalp donor area and FUSE grafts are taken from the scalp donor area above and below the strip as well as from other robust beard and body donor areas.

More about Body Hair Transplant in patient with poor scalp donor hair quality

Since our last post regarding use of chest and beard hair in patients with very limited and sparse/thin scalp donor, we have been requested to share picture of the patient’s scalp donor hair quality/characteristics.

In the following picture, we (with the patient’s permission) cut two hair from patient’s scalp donor area. The scalp hair is placed in the extreme left corner for sake of comparison.

As can be seen, the scalp donor hair in this patient is extremely thin in caliber compared to his beard and chest hair and can not provide good coverage.

Traction literally means “pull”. In India, this sort of preventable hairloss is commonly seen in the Sikh community. The simple reason behind it is hair being pulled into a tight knot before tying the turban. The tight knot exerts pull/traction especially on the hair in front and temples/sides as can be seen in the picture.

A very simple way of preventing this hairloss is to tie a comparatively loose knot so as not to put traction on hair.

Pattern hairloss (or common baldness), is the bane of a large part of the population, men and women alike. Moreover, hair thinning becomes obvious only when more than 50% of the hair have succumbed to the deleterious effects DHT (dihydrotestosterone), responsible for pattern hairloss.
